The DUTCH Test: A Deeper Look at Your Hormones

Your hormones are a conversation happening inside your body every single day. The DUTCH test lets us actually listen to that conversation — not just catch a snippet of it.

Standard blood hormone testing gives us a snapshot. It tells us what's circulating in your bloodstream at one moment in time. But hormones are dynamic — they rise and fall throughout the day, they get processed through your liver, and the way they're broken down matters just as much as how much of them you have. That's exactly where the DUTCH test goes further.

DUTCH stands for Dried Urine Test for Comprehensive Hormones. It's a functional testing tool I use in clinic to get a much more complete picture of what's really going on with your hormonal health — particularly for women experiencing symptoms that don't fully add up on standard bloods.

What does the DUTCH test actually measure?

Rather than a single blood draw, the DUTCH test uses dried urine samples collected across a 24-hour period — typically four to five collections throughout the day and into the evening. This captures the natural rhythm of your hormones, not just one data point.

SEX HORMONES: Oestrogen, progesterone, and testosterone — including how they're produced and how your body is breaking them down.

OESTROGEN METABOLISM: How your liver is processing oestrogen through Phase I and Phase II detoxification pathways — this matters enormously for hormonal balance and long-term health.

CORTISOL & CORTISONE: Your complete cortisol rhythm across the day, revealing how your adrenal glands are actually functioning — not just whether you're in or out of range.

DHEA & ANDROGENS: Adrenal androgens including DHEA S, which play a key role in energy, libido, mood, and resilience.

MELATONIN: An overnight creatinine-adjusted marker of melatonin production — helpful for understanding sleep quality at a deeper level.

Who might benefit from DUTCH testing?

In my clinical experience, the DUTCH test tends to be most valuable when someone has symptoms that point to hormonal dysregulation — but standard bloods haven't given a clear answer, or the pieces just aren't adding up.

COMMON REASONS WOMEN COME TO ME ASKING ABOUT DUTCH TESTING

  • Fatigue that doesn't improve with rest Heavy, painful, or irregular periods

  • Poor sleep or early waking

  • PMS, mood changes, or irritability

  • Hormonal acne or skin changes

  • Anxiety or low mood

  • Brain fog or difficulty concentrating

  • Low libido

  • Perimenopause symptoms

  • Weight changes, especially around the middle

WHY OESTROGEN METABOLISM MATTERS SO MUCH

This is one of the things I find most valuable about the DUTCH test, and it's often completely overlooked in conventional testing. When your body metabolises oestrogen, it can go down several different pathways — some of which are more favourable than others.

The 2-hydroxy oestrogens are generally considered protective. The 16-alpha pathway tends to be more proliferative. And the 4-hydroxy pathway, particularly when not properly detoxified, can be problematic for long-term hormonal health. But here's what's really important: even if your oestrogen levels look normal on a blood test, if your detoxification pathways are imbalanced, you can still experience oestrogen-dominant symptoms — heavy periods, breast tenderness, mood changes, fluid retention — and potentially carry a higher risk profile over time.

The DUTCH test shows us exactly which pathways are dominant, and whether your Phase II liver detoxification (particularly methylation) is working well enough to clear those metabolites efficiently. This is information we can actually act on through targeted nutrition, lifestyle support, and naturopathic supplementation.

A NOTE ON CORTISOL PATTERNS

The DUTCH test captures both free cortisol (what your body is actually using) and total cortisol metabolites (how much your adrenals are producing overall). This distinction matters. You can have normal-looking total cortisol but a dysregulated diurnal rhythm — meaning your cortisol might be low in the morning when you need it most, and higher at night when it disrupts sleep.

This pattern is extremely common in women experiencing burnout, and it's completely invisible on a standard cortisol blood test. It's also possible to have high metabolised cortisol (your adrenals working overtime) with low free cortisol — a sign of tissue-level cortisol resistance or accelerated clearance. Knowing which pattern you have completely changes the support strategy.

What happens after the test?

Receiving a DUTCH report without guidance can feel overwhelming — it's detailed, and interpreting the patterns correctly takes clinical training and context.
